Phyllis Cook, 86, of Platteville, died on Thursday, Oct. 31.

Funeral services will be at 1 p.m. on Friday, Nov. 8 at the Melby Funeral Home & Crematory, Platteville. Pastor Mary Ann Floerke will officiate. Burial will be at Hillside Cemetery, Platteville.
